To register for Mason Hockey try-outs:
FOR NEW PLAYERS ONLY:
1. Complete the team's recruitment form, which will be electronically submitted directly to the coaching staff. You are encouraged contact Head Coach Steve Hyjek via email at shyjek@bakerdonelson.com or text him at 202-309-3003 to confirm our receipt of your try-out form.
2. Download, print out and sign the GMU Ice Hockey Policy Agreement form (at the bottom of this page) and bring it with you to the first day of try-outs.
FOR ALL TRY-OUT CANDIDATES
· All try-out attendees will need to bring two checks with them to the first day of try-outs. A deposit check of $1,000.00 (made payable to George Mason University) must be brought to try-outs, along with the signed Hockey Policy Agreement noted above. The check you provide represents a deposit for 2019-20 season dues and will only be deposited upon making the team. All players will also need to provide an additional $50 try-out fee which will be credited to your dues if selected for the team.
· Please arrive to the first day of try-outs between ONE HOUR BEFORE SCHEDULED ICE TIME (TBD).
· There are online forms that must be filled out by each player, both new and returning, prior to stepping on the ice. Bring a copy of each form you have completed as part of your registration.
**ALL PLAYERS: These online forms can be filled out starting after August 17th, to make the process on the first day of try-outs move quicker. Please contact Cam Helfeldt (443-883-6713) via text for more information on how to do this. Include your full name at the beginning of the text.**
· If you have not completed your forms before August 24th, arrive one and a half hours before the scheduled ice time – a limited number of laptop computers will be provided for you to do so.
· Also, a physical must be completed using the GMU Club Sports physical document: http://recreation.gmu.edu/wp-content/uploads/2012/11/Mason-Club-Sports-Student-Physical-Form.pdf. NO OTHER FORM WILL BE ACCEPTED. The form must be completed by a doctor prior to try-outs and brought with them on the first day. This is for all players; a new physical form is required every year.
· Please bring proof of medical insurance as well – this is required to participate in try-outs.
· For new players needing transportation from the Fairfax campus to the Prince William Ice Center, please contact Cam Helfeldt (443-883-6713) or Coach Hyjek (202-309-3003) with your full name included in the text and we will coordinate transportation for you.
After try-outs / prior to 1st Practice
· Each player must complete a concussion test administered by GMU Club Sports – info on scheduling / location of concussions test can be found at https://recreation.gmu.edu/club-sports. This is a PLAYER RESPONSIBILITY to complete this test prior to first practice.
· A mandatory team meeting will occur at the Recreational and Athletic Center (RAC) on the Fairfax campus on Tuesday, August 28 from 7:30PM - 8:30PM prior to our first practice on Wednesday, August 29. At that meeting we'll confirm that all forms and concussion tests are completed, as well as collection of the balance of your dues.
· No rostered player can practice until dues are paid, forms completed, and GMU is satisfied with your physical / concussion test results.
· If you are not selected for the roster, please attend the first team meeting so that you can complete a form to have the deposit you provided reimbursed to you.
